N-Acetyl-Cysteine (NAC):

  • Mechanisms: NAC is a precursor to glutathione, a critical antioxidant that protects both liver and kidney cells from oxidative damage. In the liver, it supports detoxification by conjugating toxins and reduces inflammation through its antioxidant effects. For kidney function, NAC minimizes oxidative stress in the renal tubules, which may reduce acute and chronic kidney injury.
  • Evidence: NAC is the standard treatment for acetaminophen-induced liver damage due to its ability to replenish glutathione and prevent hepatocyte damage. In liver conditions like NAFLD and chronic hepatitis, studies have reported reductions in ALT and AST levels. For kidney health, clinical trials suggest benefits in mitigating contrast-induced nephropathy and reducing biomarkers of kidney injury in sepsis and other conditions.

Dandelion (Taraxacum officinale):

  • Mechanisms: Dandelion has antioxidant properties due to compounds like luteolin and chlorogenic acid, which reduce oxidative stress and inflammation in the liver. It promotes bile production, contributing to liver detoxification and fat metabolism. For the kidneys, it has mild diuretic effects, supporting enhanced excretion of waste products and potential prevention of kidney stone formation.
  • Evidence: Animal studies demonstrate dandelion’s ability to protect against hyperlipidemia-induced liver damage and reduce elevated liver enzymes. Limited but encouraging in vitro and animal evidence suggests renal benefits, particularly concerning antioxidant and diuretic effects, though clinical studies in humans are sparse.

Milk Thistle (Silybum marianum):

  • Mechanisms: Silymarin, the active compound in milk thistle, inhibits lipid peroxidation, reduces free radicals, and stabilizes cellular membranes, protecting liver and kidney cells. It also modulates anti-inflammatory pathways, decreasing cytokine production in both organs.
  • Evidence: Clinical trials indicate milk thistle reduces ALT and AST levels in liver diseases, such as NAFLD and alcoholic hepatitis. Animal studies show protective effects against nephrotoxin-induced kidney injury, and silymarin has been shown to reduce markers of oxidative stress in renal tissues.

Schisandra (Schisandra chinensis):

  • Mechanisms: Schisandra’s lignans enhance liver enzyme activity involved in detoxification, including glutathione synthesis. It also reduces oxidative stress and inflammation in kidney tissues, improving renal function under stress conditions.
  • Evidence: Studies in humans and animals show that Schisandra improves liver enzyme profiles and hepatocyte regeneration. Its effects on kidney health are supported by animal studies indicating enhanced renal antioxidant defenses and protection against toxic kidney injury.

Chanca Piedra (Phyllanthus niruri): aka Stone Breaker:

  • Mechanisms: Chanca Piedra inhibits oxidative stress in hepatocytes and promotes liver detoxification by supporting bile flow. For kidneys, it has potent anti-lithogenic effects, inhibiting stone formation, and reducing inflammation and oxidative injury in renal tissues.
  • Evidence: Animal and in vitro studies show reductions in liver enzymes and attenuation of fibrosis in liver injury models. Clinical and preclinical studies demonstrate efficacy in dissolving kidney stones and improving kidney function in nephrotoxic states.

Uva Ursi (Arctostaphylos uva-ursi):

  • Mechanisms: Uva ursi contains arbutin, which is metabolized into hydroquinone, providing diuretic and antimicrobial effects that benefit the urinary tract and kidneys. Its antioxidant compounds reduce inflammation and oxidative stress, indirectly supporting liver health.
  • Evidence: Limited research directly links Uva Ursi to liver support, but its diuretic and anti-inflammatory actions may benefit liver detox pathways. For kidney health, it has traditional and clinical support for treating urinary tract infections, reducing inflammation, and promoting renal function.

Bupleurum (Bupleurum chinense):

  • Mechanisms: Bupleurum modulates liver enzyme activity, reduces oxidative damage, and has anti-inflammatory properties through inhibition of pro-inflammatory cytokines. It helps prevent renal damage by reducing oxidative stress and inflammatory damage in kidney tissues.
  • Evidence: Numerous animal studies support Bupleurum’s ability to lower liver enzymes and protect against hepatotoxic injury. Preclinical evidence suggests protective effects on kidney tissues in ischemia-reperfusion models and toxin exposure.
